Congregate Living Health Facilities are medical care facilities based in Los Angeles, California. Our company provides excellent inpatient care in a home-like, communal environment.

Who We Are

We work with hospitals and major insurance companies for our services. While we do not provide long-term acute care on the level of an intensive care unit or an acute hospital, we can assist patients who need help with:

  • Sub-Acute Rehabilitation
  • Ventilator or Tracheostomy Dependence/Complex Pulmonary Care
  • Respiratory Therapy Services for Oxygen Delivery, Suctioning, CPAP or BiPAP
  • Dietary Care Including Tube Feeding or Total Parenteral Nutrition (TPN)
  • Gastrostomy or Ileostomy Care
  • Catastrophic Illness
  • Chronic Neurocognitive Deficit
  • Complex Wound Care
  • Med Monitoring of IM Antibiotics
  • Psychosocial Support
  • Disabled Persons Requiring Assistance With ADLs, IADLs, and/or Medical Skilled Needs
  • Also Includes Patients With:
  • Advanced Stage Neurological Disease
  • Quadriplegia, Hemiplegia, or Paraplegia
  • Severe Neurologic Illness
  • Dependence on Medical Technology With Other Comorbidities

What We Do

We offer a wide range of medical services 24 hours a day, seven days a week. These include:

  • Continuous (74-hour) skilled nursing and supportive care provided by licensed healthcare professionals under the supervision of a board-certified medical director
  • Ventilator or tracheostomy dependence/complex pulmonary care that cannot be provided at a lower level of care
  • Intermittent respiratory services for oxygen delivering/suctioning, CPAP or BPAP
  • Dietary care, such as feeding or total parenteral nutrition with other medical co-morbidities
  • Gastrostomy or ileostomy care
  • Complex wound care (debridement, packing, extensive dressing) requiring an extended period or intravenous analgesia prior to care
  • Medication monitoring of intravenous antibiotics or five or more medications
  • Frequent, intermittent vital signs, neurologic, and/or vascular monitoring requiring 24-hour care
  • ADLs, IADLs and/or medical assistance to physically disabled persons
  • Psychosocial support provided by licensed clinical social workers and therapists
  • Subacute rehabilitation for physical, occupational, and recreational therapy
  • Skin care, oral care, and position changes that cannot be provided at a lower level of care
  • Prior authorization requests may be initiated by the members healthcare team, which can be at short-term and long-term acute hospitals or a traditional subacute care facility

Any requests will be approved or evaluated based on your medical necessity.
